Provides transitional housing and supportive services for homeless veterans with serious mental health and substance abuse problems. Helps each veteran achieve greater self-determination, improved residential stability, and increased income and/or job skills. Provides safe and affordable housing, develops an individualized service goal / treatment plan for each veteran, and provide case management services. Helps end veteran homelessness by providing housing stability to homeless and at-risk veterans and their families. The goal of the program is to promote housing stability among very low-income Veteran families who reside in or are transitioning to permanent housing. Support outreach, case management and other flexible assistance to rapidly re-house Veterans who become homeless or prevent Veterans from becoming homeless. Eligibility: Eligible veteran is a person who served in the active military, naval or air services and who was discharged or released under conditions other than dishonorable. A Reservist can also be considered eligible if activated during ""War time"" or in a ""State of Emergency"" and deployed out of State.
