The Bucks County Emergency Homeless Shelter is a temporary residence for individuals and families who have lost safe and stable housing. Here, they offer case management; job search and preparation resources; health evaluations, screenings and referrals; mental health and substance use referrals; and transportation to doctors’ appointments, job interviews, and summer camp for children. They offer housing first blended case management for individuals who have been chronically homeless and in need of obtaining and maintaining housing. They provide services to eligible adults 18 and older who also may be struggling with mental and/or co-occurring disorders. Tenant based Rental Assistance Funding will not be available to individuals after age 62. They offer housing referrals, help with housing costs (move in costs, security, deposits, rental applications, rental assistance), integrated treatment for mental health and/or substance use disorders, connection to other community resources, service planning and coordination, and empowerment and self-help. All referrals must include a completed HF-BCM referral form, as well as the referral form completed in HMIS. Referral will not be complete without insurance verification of eligibility. A referral may come from a street outreach team, the Bucks County Emergency Homeless Shelter or Bucks County BH/DP.
