Provides service coordination for children under the age of three with developmental delays or disabilities. They work in partnership with parents and early intervention specialists to evaluate each child’s needs, explore options, and develop an individualized family service plan for the child and family. Services include special instruction; physical, occupational, and speech therapy; social work services; psychological services; hearing and vision services; nutrition services; nursing and other health services. Eligibility includes: children aged 3 or under who are living with a developmental delay or disability.
