Public Health Management Corportation – Delaware County Reentry Project

Aims to address challenges with the reentry process for individuals released from the George W. Hill Correctional Facility. They support individuals before release from incarceration and through their reintegration into the community. This is a voluntary program, with services available for a period of 60-90 days both before and after release. Services include reentry planning to assist with the transition from incarceration; connections to resources for employment, behavioral health, and housing; strengthening of relationships between incarcerated parents and their children; and support to build positive, beneficial relationships with the community. Eligibility: any individual who is currently under the supervision in the community, any individual who is serving a sentence for a new conviction and/or violation of probation/parole, there are currently no exclusions regarding criminal offense(s), and Delaware County resident (and Philadelphia residents on a case-by-case basis).
